Rectangular paper form with rounded edges. Attached to the front is an image of a nun and a piece of cloth with red stitches. Below the image and the piece of cloth “Dr. Edith Stein” is written in black ink. Another rectangular paper form is adhered to the verso. This form has an image of a crest.
